The effectiveness of root cause analysis (RCA) brought Action Services partners, Bill Gilchrist and Dana Mosher together to form the company in 2003. Bill had been working as a reliability engineer and root cause consultant in Virginia and Dana was busy implementing RCA in a major labor organized pulp and paper manufacturing mill in Maine. " North met South" through training opportunities offered by Dana's employer and after several such by chance meetings the two began an ongoing dialog about the kinds of issues that made RCA work in real life, what parts of RCA that became "discouraging" to those working with it and about the existing processes that were "unnecessarily complicated" and difficult to learn. Teaching about RCA and actually getting it implemented into a workplace so that it just becomes "the way we do business" was quite another matter. Each work site has its own culture. They are like finger prints; no two are alike. We felt that an effective RCA needed to be built on employee involvement, a commitment to find solutions, not blame or to punish, a respect for different work cultures, and quite simply, to keep the process "simple". We believe in the old K.I.S.S. principle other wise known as the Keep it Simple Stupid process.

We believed that by combining our real life experience with RCA, the scientific method, and our experience in facilitating RCA teams, we could develop a method that would be the "One Minute Manager" of RCA. A method that would be easy to read, easy to learn, easy to implement, and most importantly – VERY EFFECTIVE!

After a lot of soul searching about our own career direction and frankly just what we wanted out of life, the time just seemed to fall in place for us to develop our ideas and create Action Services. Call it destiny, call it the higher power, call it whatever you want, it is strange how two strangers can meet by chance and end up blending their experiences and expertise into a new company. It's the great American way!
